If you are sending someone else to shop for you and they need to use your Owner Tab, you need to fill out this form to authorize that person to use your Tab. They will not be able to use your Owner Tab without permission from you via this form. This is a one-time-only permission, and every time anyone will be shopping in your place and using your Owner Tab you need to fill out the form again.
